ADD DEPENDENT/UPGRADE

 

ALL  upgrade additions are assigned a $3 change fee, a prorated amount, and an enrollment for dependents.   We highly recommended stopping in during our staffed hours when making additions as it can take 3-5 business days to issue the upgrade through this web form and take payment.  Also, when adding a dependent they will still need to stop by to sign a liability waiver, take an account picture, and receive an access card.  PLEASE READ any confirmation email closely as it does contain detailed information about your changes.  To proceed click the grey button below and/or refer to your membership agreement for more details.
